PHYSICAL EXAMINATIONS 

N.J.A.C. 6A:16-2.2 & N.J.S.A. 18A:40-4 requires that each student, upon entry into the school district, shall have a medical examination conducted at the medical home (healthcare provider) of the student, and a report sent to the school nurse within 30 days of new enrollment. The complete physical examination shall include the immunizations, medical history including allergies, past serious illnesses, injuries and operations, medications and current health problems, health screenings including height, weight, hearing, blood pressure and vision. This examination must be completed no more than 365 days prior to school entry and must state what, if any, modifications are required for full participation in the school program (see document: Student Medical Examination Form).

Recommended subsequent medical examinations shall be conducted at the medical home (student’s healthcare provider) and a report sent to the school nurse at least one time during each developmental stage at early childhood (preschool through grade three), pre-adolescence (grade four through six), and adolescence (grade seven through 12). These examinations should be conducted at the child’s medical home in order to ensure continuity of care (see document: Student Medical Examination Form).

TUBERCULOSIS SCREENING

Tuberculosis screening is required for students entering the school district from specific foreign countries determined by the State of New Jersey Department of Health, as per state directive. Your private healthcare provider or local health department can perform this test. Documentation of Tuberculosis screening must be provided as soon as possible. 

New Jersey Department of Health Tuberculosis Program Recommended Tuberculosis Testing in New Jersey Schools

1. Only a positive interferon gamma release assay test result or a Mantoux intradermal test using 5 TU of stabilized PPD tuberculin skin test result measuring > 10mm of induration shall be considered a “significant reaction” and evidence of latent TB infection.

2. The following tuberculosis testing recommendations apply to ALL school districts:

1. Students born in a country EXCLUDING THOSE COUNTRIES listed below and entering school in the U.S. for the first time, regardless of age or grade will require TB testing results to be submitted to the school nurse.

2. Students transferring into the New Jersey school system directly from a country EXCLUDING THOSE COUNTRIES listed below regardless of age or grade will require TB testing results to be submitted to the school nurse.

EXCEPTIONS FOR BOTH GROUPS OF STUDENTS LISTED ABOVE: 

• Entering at preschool through grade 5: Tuberculosis testing is not recommended if the student has documentation of an IGRA or Mantoux tuberculin skin test at the age of three years or older, regardless of the result of that test. 

• Entering at grades 6 through 12: Tuberculosis testing is not recommended if the student has documentation of a negative tuberculosis test in the last six months or a positive test, regardless when this test was done. 

• Tuberculosis testing is not recommended if the student has attended school in another state prior to entering the New Jersey school system.
